There are two ways to register:
1. Download this form, fill it out, and mail it along with your check (payable to Dallas Woodburn) to:

2. Fill out the camper's name and contact information below.
In the "Message" section, be sure to include:
- Your mailing address,
- The camper's grade level and school
- The dates and session (morning or afternoon) the camper will be attending.
You can mail a check to the above address or bring a check to the first day of camp.
(NOTE:
Your registration will not be fully processed until payment is
received. If you choose to pay on the first day of camp, you will not
be eligible for the Early Registration Discount.)
